EXPERIENCE Veterans
Affairs Medical Center SLC Carpenter
Supervisor (Construction Foreman) 2010 to 2013 Provided on-site
supervision of a very important remodel of the Surgical Suite at the Veteran
Affairs Medical Center in Salt Lake City. The space consisted of 6 Operating
Rooms as well as the Clean Core storage area and other space which supports
the Operating Rooms. Over the course of the 3 year project the existing space
was completely gutted. The project was constructed in 2 phases as to allow
continuing surgeries as needed for our Veterans. The Engineering Dept. at the
VA made the decision to utilize the VA’s Purchase & Hire program
(P&H) to build this project. P&H allows the VA to hire construction
crews on a temporary basis in order to complete certain projects. Our crew
consisted of Carpenters, Plumbers, Electricians, and HVAC personnel. Duties
as the P&H Supervisor were typical of a commercial construction
superintendent and were: · Leading subs, suppliers, and P&H crew in the
full range of field engineering and construction activities. · Monitoring quality of work performed by the P&H
crew as well as subcontractor workmanship. · Reading and interpreting the plans &
specifications. This includes finding the mistakes in the plans and
specifications and then working with subcontractors and/or suppliers to
create solutions as to why the vague or incorrect plans and specs should not
result in a change order. If change order was justified, participate in the
process by giving clarification to Construction Control Rep. and Contracting
officials. This could include providing field measurements, pointing out
irregularities in the plans and specifications and having first hand
information of site conditions. · Conducting quantity take-offs from plans and
specifications and ordering project materials and scheduling deliveries of materials.
This was important due to a lack of storage space at the VA. · Work with subcontractors and suppliers to
accommodate their proposed work schedules. · Performing daily job-site surveillance to protect VA
property, personnel, materials, tools, and offices. · Scheduling and conducting weekly construction
meetings with customer, subs, and suppliers. · Keeping and recording P&H man-hours and
reporting time cards to the time keeper on a weekly basis. · Keeping daily logs of project activities and crew,
subcontractor, and supplier activities. · Working on and submitting red line As-Built drawings
and specifications. · Applying practical knowledge learned from years in
the industry. · Confidence to make decisions in the field which will
allow the project to continue but will not create major changes of the plans
and specs. Examples of these types of decisions are changing the swing on a
door. Relocating duct work so to create better use of space above ceiling.
Moving a sink to create better drainage. Changing the orientation of troffer
lights. · Supervised the demolition and installation of new
and existing Medical Gases. These included O, N, C02, Nitrous Oxide, Medical
Air, Medical Vacuum, and Waste Anesthesia Gas. Hoffman
Construction Co. CSA Engineer (Project Engineer, Project
Administrator) 2005 to 2007 Worked on completion of the Micron facility In Lehi,
Utah. Duties surrounded the Civil, Structural, and Architectural areas of the
project. Duties as the CSA Engineer were consistent with typical Project
Engineer and/or Assistant Project Manager duties within the construction
industry and were: · Performed all aspects of a Project Engineer within
Civil, Structural, & Architectural disciplines. Managed all subcontractor
submittals, shop drawings, RFI’s, O&M’s and kept and maintained each log.
· Coordinated required City inspections for each
Hoffman subcontractor. Walked with Lehi City Inspectors on all inspections. · Kept inspection log and worked with subs and
suppliers on completion of deficient items. · Because of prior clean room experience, I worked
very closely with the Hoffman clean room superintendent and Architect to
ensure an “install it once” clean room ceiling grid and raised metal floor. · Assisted the CSA Manager with subcontract
management. · Managed and maintained an SOR log of over 3800 punch
list items. · Conducted critical CSA workgroup meeting to
coordinate workload and resolve issues. Attendees were Superintendents,
Project Manager, Owners Representatives, Engineers, Architects, and Schedulers
as well as anyone else who needed to attend. · Walking jobsite and conducting inspections with
subcontractors to ensure proper construction is occurring per plans and
specifications. · Worked closely with the structural engineers and
owners representatives to value engineer solutions. DawTech Assistant Project Manager (Project Engineer,
Project Administrator) 1997 to 2005 Provided onsite
management of Automated Material Handling Systems (AMHS) installations for a
large Daw Technologies client who is a supplier to Intel. Intel manufacturing
facilities include Fabs D1D, D1C, RB1 (Hillsboro, OR) and Fab 24,
(Leixlip, Ireland) AMHS installations are for Intel’s 300-mm technology and
include stockers and inter bay clean way systems. · Staffing projects with appropriate labor force
utilizing both union and non-union labor. · Mobilize and demobilize project. · Coordinate project start-up with client and Intel
personnel, including submission and Intel approval of Job Hazard Analysis,
Installation procedures, Fall prevention/protection plans, Open floor hazard
plans, and use of daily safety Pre-Task planning worksheets. · Train the Daw onsite labor forces to Intel’s
extremely high safety standards and instill Intel's “culture of safety” into
crews. · Coordinate material deliveries with client and work
with other trades inside of Fab on a daily basis to create an uncluttered
workspace. · Daily update of client schedule coupled with
pre-planning for following day’s activities. · Train labor force as to proper clean room protocols
and standards. · Weekly tracking of man-hours and materials and
